| Form 6.02c - Release and Waiver of Liability: Cancellation of Bus Routes/Extended Waiting Periods - To be completed by parents of students who ride “center-to-center” buses. These are buses transport students to particular schools and programs using schools as bus stops. |

| Form 6.03b - Request for Special Transportation Accommodation - To be completed by parents who want to request a special transportation accommodation specifically permitted in Chapter 6 of the School Board Rules. When the transportation accommodation involves “space available” transportation, Form 6.04 Space Available Transportation Acknowledgment is also required. |

Form 6.04c - Space Available Transportation Acknowledgment - To be completed by parents after a request for special transportation has been approved and a bus ticket has been issued. This is not a transportation request, it is an acknowledgment that, if granted, space available transportation can be withdrawn by the District as described in the acknowledgment.

Note: Before completing the Bus Stop Change Request, please click on the Determining Bus Stops link in the left margin. While rules don't always cover the "gray areas," determinations regarding the suitability of bus stops are primarily governed by Florida Statutes and School Board Rule. In planning bus stops, we consider State Law, Board Rules, and
1. The safety of the bus stop.
2. The ability of the bus stop to serve nearby residences with maximum efficiency (time, taxpayer money).
3. The impact on other students served by the bus stop.
With the exception of center-to-center (C2C)stops, the Transportation Department is also guided by Florida Statutes in terms of the maximum distances from home to the bus stop and by the need to avoid having children walk across four lanes of traffic through medians greater than five feet in width.
Please remember that School Board Rule requires that parents take full responsibility for the safe movement of their children between the home and the bus stop and their safety at the bus stop while awaiting bus transportation. Therefore, if parents are concerned about their children's ability to walk safely from home to the bus stop, they should make necessary arrangements to ensure their safety.
